Please contact us to inquire about your insurance in more detail. We will verify your benefits and will let you know if you have any co-pays, deductibles or coinsurance. If after we review your benefits we determine that we are not in-network with your specific insurance plan we can look to utilize your out-of-network benefits if you have them or we will discuss sliding scale/self-pay rates.
